The Olmos Park Dental Membership Plan is designed to provide affordability and greater access to quality dental care. You will receive immediate, hassle-free, and convenient eligibility. Dental services will be provided by the Olmos Park Dental team. It is not a dental insurance or benefit plan. This plan may be enhanced as our services progress in advancement of quality care. Patient portion is due at the time of the services.

- First Family Member $395

- Each Additional Family Member $245
- You have the option to invest in your membership plan upfront annually with automatic renewal or set up automatic monthly payments.
- Note: All family members must live in the same household or be a legal dependent.
TERMS & LIMITATIONS OF THE PLAN
- It is good only for Olmos Dental. If you are referred to a specialist, they will not offer this discount.
- Should there be dental treatment needed following any type of injury where a lawsuit and therefore outside medical care, care, disability or workman’s comp type insurances are involved, this discounted plan cannot be used.
- This plan is non-transferable – family members cannot be substituted for another family member.
- It is nonrefundable – no refunds are given if the patient chooses not to use their dental plan.
- Payments for services are due at the time of service.
- This offer cannot be combined with any other offers, insurance, or benefit plans.
- For orthodontic treatment, the participant must remain a plan participant for the entire duration of orthodontic treatment.
- Dental services only.
- Membership plans are based on an annual contract and will be renewed automatically unless canceled by the member.
- Monthly memberships canceled prior to their full annual length will be charged the remaining amount at the time of cancellation.
